Awards selection criteria
A nominated individual may be a caseworker, supervisor, administrator
or any other individual who works directly for a Colorado county.
A nominated unit may be a functional unit, such as an intake or
program unit, or some other form of unit as defined by a Colorado
county.
The nominated individual or unit has demonstrated commitment to
the core outcomes, and has made outstanding contributions in one
or more of the outcome areas – Safety, Permanency and Well-Being
(see About the EIP Awards for definitions).
The nomination requires basic contact information for your nominee,
his or her supervisor, and yourself. You will also be asked to respond
to the following questions. We recommend that
you write your responses in another program, such as Word, and then
cut and paste them into the form. Each question will be limited
to 750 total characters (including spaces). This equals approximately
150 words per question.
1. Describe your nominee’s achievements,
accomplishments, and contributions on behalf of children, youth,
families and the community.
2. Explain how your nominee is a role model for Child Welfare practice.
3. List specific examples of how your nominee goes above and beyond
his or her job’s expectations.
4. List specific examples of innovative practice, vision, leadership,
independence, commitment or other qualities that demonstrate your nominee's excellence.
5. How has your nominee impacted programs or service delivery within
his or her county or department?
6. How have these factors influenced one or more of the core outcomes
of Safety, Permanency and Well-Being?
